    1. Where have you had the best success with ad placement in relation to text?

    Examples:

    header
    ad box - content
    contnet content (content wrapped around adsense box)

    OR

    header
    ad
    content
    ad
    content

    2. Do you have better success with ads blending in or sticking out?
    3. How close can I put an image to an ad unit or link unit?

    1. I think it's totally depend on your blog style. I prefer Header - Ads - Content - Ads. Each blog get different result I think.
    2. It's better to make ads as usual text, I mean lot of people is ads minded, So They won't click if They regard the link as ads.
    3. You can put in image quite close as long as You give a border on your adsense ads.
