    I placed 3 ad formats on each page of my site. (Not the one in my sig)
    On some pages, all 3 ads appear. On others only one ad is displayed.
    Why is this so?

    This is very common. On some subjects (for some keywords), Google does not have enough ads to populate all your ads space.
