It looks like u havent given proper info or given some wrong info while filling the form
go to google.com/adsense and log in with u r id and resubmit u r apps !
hope this will resolve u r poblem

 

Obviously all you need to do is change the Payee name field. Change the name field to your name, because you are the one that Google will sending cheques to.
