go for drupal, you can make as many hierarchal structures as you can.
Here is an example site: http://alztudio.com - using drupal of course
Wordpress can also do that but its a blogging platform.

I've installed and tried to use Drupal at least 5 times... I just can't seem to wrap my mind around it. Very easy to install, beautiful code, and clearly very powerful. But it just hasn't really "clicked" in my head yet. I'll probably try again, because I do like the package.

Tried Joomla in the past. Seemed much easier to use than Drupal, but there was something I didn't like about it. Sadly, I don't remember exactly what that was.

just to shoot another CMS at you.. have a look at

    www.ccbriefing.co.uk
    www.resa.es
    www.lbg-online.net
comunicalia.cometatech.com <<to be launched really really soon

these are all sites I built using ezPublish and i've built many many more.. :D

The example (yourtotalhealth.ivillage.com) you mentioned would definitely be a can do with ezPublish
