Which contact form do you use?

Hello!

I need advice from webmasters: which contact form do you use?
Do you use a software for your contact, a script that you bought from the web, ect?
Actually my WBS website use the Super Ajax Contact Form from http://codecanyon.net/
But the script doesn't work perfectly, i want to use another script but don't know which.
I want a very simple contact form (php, html or java, ajax) like this one: http://www.quantumseolabs.com/contactus

I Use aweber.com. They cost depending on how many people are on your list but its well worth it.

Edit: Just read again, you need a contact me form. I use one same type as above with a captcha code to prevent spam.

However Aweber won't help me because it is for newletter.

 

I use a simple PHP script I wrote that does its' best to weed out junk and then emails the message to the appropriate email address. When people have asked me this question I've pointed them to http://www.emailmeform.com/ and all seem to be happy with that.
